Requirements:

  1. Complete the online application – make sure current phone contact information is provided.
  2. Provide your DD 214 form and copy of Drivers license or State ID as an email   attachment
  3. Submit online application
  4. Send attachments with the subject: application attachments, and your name to this link:  appattach@saveavet.us 

A Save A Vet clinical intake representative will attempt to contact you within 24 hours.
